 How to schedule your DOT-Medical Exam when you are in Boise area?

1. You can schedule your appointment by calling during day hours to (208) 576-4696, or by going to our website appointment link (link to scheduling app). You will be prompted to reserve your appointment time and will be able to pay to reserve your DOT-exam time.

2. If your employer pays for your DOT Bi-annual physical have them call us to schedule and pay for your test before your appointment day. They can schedule the appointment for you, or you can call after payment to schedule your appointment.

3. If you are a motor carrier manager and wish to schedule a test for 10 or more in your facility, truck stop or event please call for special pricing and information.

What to expect the day of the physical test
Come comfortable and expect to change into exam gowns requested by the doctor needed. The exam process is simple and convenient. Dr. Xavier will make sure you are comfortable during the exam process. He will also make sure to accommodate to your specific needs following the rules and governing laws for this testing.


Exam procedure

Expect to spend 45-60 minutes the day of the exam in most cases. In some rare instances more time or another appointment will be needed to complete all the requirement for the certification. This will be specific to the case and individual needs of the examinee.
1. First You will be greeted and instructed on how the test will be conducted the day of the test.

2. Our staff will gather all the verification needed to perform the test. Make sure you bring your CDL license in good standing or your DL if you are taking the exam for your initial CMV driver license. We will not be able to conduct the exam if you do not have the official valid license with you. No fee will be returned, but you may have to re-schedule.

3. You will fill the registration and history form and pay for your exam. The exam must be paid in full before the doctor sees you. The form will be inspected for any error by the doctor assistant. The doctor will not be able to perform the rest of the test if you have missing information, blank or unclear areas in your test. You can download the form by pressing this link and fill it at home to save valuable time.

4. The doctor assistant will conduct the initial biometric and monitoring measurements. Your height, weight, vital signs, oximetry and blood pressure will be measured and recorded. Other test may be needed as part of the process of certification that may be pertaining to your specific needs. This may be at the sole discretion of the physician performing the exam.

5. Vision and hearing test will be performed to the national standard by the doctor assistant. In some specific instances, the doctor may require additional testing at drivers’ expense if the patient fails the basic requirements. This in the effort to help the driver comply to the standard and federal or state requirements.

6. Urine sample may be provided by the driver to the assistant. This urine will be tested on site for Specific gravity, Blood, Glucose and Proteins. This will give the doctor a general impression of the metabolic health of the driver. It will also serve as early detection of severe medical problems that can impair driver ability to perform the task associated with the commercial operation of a vehicle, or prompt the doctor to perform additional medical testing to document the ability of the driver to safely operate a heavy transport vehicle.

7. The driver will have a consultation with Dr. Xavier. He will go over the history and all the results gathered previously during the test. Please also bring any test, waivers, exceptions, letters from doctors and medical records needed to document your ability to perform the abilities of a CMD driver. The doctor may take all this into account before making his final determination in your case. The passing of the medical exam is at the sole discretion of the examiner. He will have the final word on the certification of the driver. In some specific instances, and were provided by the federal ruling, he will help you fill out for a waiver with the FMCSA. The certification process may be for a maximum of two (2) years. In some medical conditions, or when the doctor do not feel comfortable certifying the driver for the maximum (2) year period, a one (1) year or lesser time certification may be granted. There is no discounted fee for denied, or lesser time certifications. The doctor is responsible of protecting the driver and the public above all and will not grant certificates to those impaired to perform their duties safely. Most of driver will have full certification if they are able to perform at the required level described in the 49 CFR § 391.41, see bellow in this page.

8. The doctor will sign the ME certificate if the driver passes the test. The certificate will be provided to the driver. All pass and fail test will be registered at the FMCSA website by the end of each testing day.
Drivers with physical impairments, which affect their ability to safely operate CMVs, must obtain a "variance" from their State to be approved to drive commercially. The variance document must be carried with the commercial driver whenever they are operating a commercial motor vehicle. A Skill Performance Evaluation (SPE) is a special type of "variance" required for drivers with impaired or missing limbs (e.g., a hand or finger, an arm, foot, or leg). Drivers with missing limbs, if eligible, must obtain an SPE certificate. The commercial driver must always carry the SPE certificate.

DOT / FMCSA Physical qualifications for drivers:
A person is physically qualified to drive a commercial motor vehicle if that person:
1. Has no loss of a foot, a leg, a hand, or an arm, or has been granted a skill performance evaluation certificate according to § 391.49.
2. Has no impairment of:
a. A hand or finger which interferes with prehension or power grasping; or
b. An arm, foot, or leg which interferes with the ability to perform normal tasks associated with operating a commercial motor vehicle; or any other significant limb defect or limitation which interferes with the ability to perform normal tasks associated with operating a commercial motor vehicle; or has been granted a skill performance evaluation certificate under § 391.49.
3. Has no established medical history or clinical diagnosis of diabetes mellitus currently treated with insulin for control, unless the person meets the requirements in § 391.46.
4. Has no current clinical diagnosis of myocardial infarction, angina pectoris, coronary insufficiency, thrombosis, or any other cardiovascular disease of a variety known to be accompanied by syncope, dyspnea, collapse, or congestive cardiac failure.
5. Has no established medical history or clinical diagnosis of a respiratory dysfunction likely to interfere with his/her ability to control and drive a commercial motor vehicle safely.
6. Has no current clinical diagnosis of high blood pressure likely to interfere with his/her ability to operate a commercial motor vehicle safely.
7. Has no established medical history or clinical diagnosis of rheumatic, arthritic, orthopedic, muscular, neuromuscular, or vascular disease which interferes with his/her ability to control and operate a commercial motor vehicle safely.
8. Has no established medical history or clinical diagnosis of epilepsy or any other condition which is likely to cause loss of consciousness or any loss of ability to control a commercial motor vehicle.
9. Has no mental, nervous, organic, or functional disease or psychiatric disorder likely to interfere with his/her ability to drive a commercial motor vehicle safely.
10. Has distant visual acuity of at least 20/40 (Snellen) in each eye without corrective lenses or visual acuity separately corrected to 20/40 (Snellen) or better with corrective lenses, distant binocular acuity of at least 20/40 (Snellen) in both eyes with or without corrective lenses, the field of vision of at least 70° in the horizontal Meridian in each eye, and the ability to recognize the colors of traffic signals and devices showing standard red, green, and amber;
11. First perceives a forced whispered voice in the better ear at not less than 5 feet with or without the use of a hearing aid or if tested by use of an audiometric device, does not have an average hearing loss in the better ear greater than 40 decibels at 500 Hz, 1,000 Hz, and 2,000 Hz with or without a hearing aid when the audiometric device is calibrated to American National Standard (formerly ASA Standard) Z24.5 - 1951.
12. Does not use any drug or substance identified in 21 CFR 1308.11 Schedule I, an amphetamine, a narcotic, or another habit-forming drug.
13. Does not use any non-Schedule I drug or substance that is identified in the other Schedules in 21 CFR part 1308 except when the use is prescribed by a licensed medical practitioner, as defined in § 382.107, who is familiar with the driver's medical history and has advised the driver that the substance will not adversely affect the driver's ability to safely operate a commercial motor vehicle.
14. Has no current clinical diagnosis of alcoholism.
Note: The driver can obtain from FMCSA a medical variance from the physical qualification standards in a case by case, please consult the doctor for details.
About your privacy and personal information during your DOT exam



Your information is confidential, but there are some stakeholders that need to know the results of your test. This may include your employer, the state and FMCSA. Other law enforcement agencies may request information about your examination according to the governing laws. Know you are protected, no other person may have access to your file or information, that those in direct need to know basis.

Full exam service exam fee of $88.00 cash, credit or debit. All fees are due before the testing start.
Employers may send company printed checks with the driver the day of the test, only if commercially printed checks are provided.
No hand printed checks will be accepted.
No refunds will be granted for failed test.
Full fee may be required for re-exam if driver do not meet the requirements the initial exam appointment.
Other labs, exams or medical test may be needed to document driver fitness to perform.
Those tests will have cost associated and are not covered in the exam fee.
No refunds will be granted to missing appointments.
A onetime free re-schedule waiver will be granted to those who re-schedule the appointment with 24 hours of anticipation by calling the appointment line.
ME Certificate

We will upload daily the results of our test to the FMCSA website, but you will have to send a copy of your certificate to the SDLA in your state. All commercial drivers of vehicles in interstate commerce with a maximum gross vehicle weight rating of over 10,000 pounds (4,536 kilograms) are required to obtain and maintain a valid Medical Examiner's Certificate (ME Certificate) Commercial drivers who drive vehicles requiring a CDL have two additional requirements. CDL holders must provide their SDLA with a copy of their ME Certificate. This information is only being added to the State driving records of CDL holders.

In Idaho, you can mail or email your exam certificate to the Idaho Transportation Department PO Box 7129 Boise ID 83707-1129, or medical cards can be emailed to dlmail@itd.idaho.gov
Only the wallet card is required not the long-form medical examiner’s report that we will give you for your records. Idaho’s self-certification process can be completed quickly and easily online at https://dmvapps.itd.idaho.gov/cdlselfcertification
Drivers who are required to submit a medical card to ITD may submit the card via FAX, E-mail, US Mail, or may take the card to any County Driver’s License office to be scanned. Commercial drivers who fail to self-certify, and if required, submit a medical card to ITD on or before January 30, 2014, will have their Commercial Driver’s privilege removed from their driver’s license. Drivers must also keep a copy of their medical card current with ITD to maintain commercial driving privileges. Do you need to have the certificate with you all the time?
A person subject to this part must not operate a commercial motor vehicle unless he or she is medically certified as physically qualified to do so. Drivers from any of the countries who have received a medical authorization from Canada and Mexico may also be granted reciprocity to drive in the USA. Check your local DMV and State DOT office.
A person who qualifies for the medical examiner's certificate by having obtained a medical variance from FMCSA, in the form of an exemption letter or a skill performance evaluation certificate, must have on his or her person a copy of the variance documentation when on-duty.
Recent ruling doesn’t require the driver to carry the certificate on his or her person the medical examiner's certificate specified at § 391.43(h), or a copy, for more than 15 days after the date it was issued as valid proof of medical certification.
On or after June 22, 2021, a driver required to have a commercial driver's license or a commercial learner's permit under 49 CFR part 383, and who has a current medical examiner's
certificate documenting that he or she meets the physical qualification requirements of this part, no longer needs to carry on his or her person the medical examiner's certificate specified at § 391.43(h).
Beginning on July 8, 2015, and through June 21, 2021, a driver required to have a commercial learner's permit under part 383 of this chapter, and who submitted a current medical examiner's certificate to the State following § 383.71(h) of this chapter documenting that he or she meets the physical qualification requirements of this part, no longer needs to carry on his or her person the medical examiner's certificate specified at § 391.43(h), or a copy for more than 15 days after the date it was issued as valid proof of medical certification.
A CDL or CLP holder required by § 383.71(h) of this chapter to obtain a medical examiner's certificate, who obtained such by having obtained a medical variance from FMCSA, must continue to have in his or her possession the original or copy of that medical variance documentation at all times when on-duty.
